Licensing Program Analyst (LPA) Tuoc Doan conducted a scheduled Tele-Pre-Licensing inspection via video conference call with Natalia Makarova, Director and Jeff Kamradt, Manager of Applicant Makarova-Kamradt, LLC. Applicant's Representatives were informed that due to the COVID-19 situation and "Shelter in Place" Order, this LIC809 Facility Evaluation Report will be generated at the Licensing Office and emailed to the facility. Facility’s reply to the email will serve as acknowledgement that the report was received.

Applicant Makarova-Kamradt, LLC is applying for a License to operate a Preschool Center with a capacity of 37 at 2070 Homestead Road Santa Clara, CA 95050. A Fire Clearance has been granted on 05/11/2020. The Preschool Center will operate Monday through Friday from 08:00AM to 06:00PM, serving children ages 2 years to Kindergarten. Facility's phone # (408) 368-2862.

The buildings were inspected inside and out via video conference call. Classrooms A and B and Outdoor Play Space were measured to determine the maximum capacity that meets the requirement of 35 square feet of indoor activity space and 75 square feet of outdoor activity space per child.

The measurements were taken by Natalia Makarova and Jeff Kamradt as LPA observed the process and provided direction over the video call.

There are 4 toilets and 5 sinks inside the two classrooms (A and B) for children to use. In addition, there is a restroom with one toilet and one sink across from the reception area. Children who become ill will use this restroom and will be isolated in the Office with isolation equipment. Room C is used for storage and laundry (washer and dryer). There is a restroom with one toilet and one sink in Room C for Staff to use. LPA observed soap dispenser and disposable paper towels in the restrooms. The sinks used by children have hot and cold running water. Per Jeff Kamradt there are temperature control valves under the sinks, which are currently set to deliver water at temperature no higher than 105 degrees F.

There are cubbies and hooks for children to store their belongings. Classrooms are furnished with age appropriate tables and chairs, play equipment, and toys. The facility has cots for children to use for napping.
Facility will provide Breakfast, Lunch, and Morning and Afternoon snacks. Meals and snacks will be prepared at an on-site Kitchen. LPA observed that the kitchen is equipped with stoves, a microwave, a sink with hot and cold running water, a dishwasher, and a refrigerator. There are trash cans with tight fitted lids, and paper towels and hand soap near the sink. Facility staff will clean toys and equipment daily with cleaning wipes and solutions that will be kept inaccessible to children. Detergents, cleaning compounds, sharps, medicines, and other items which could pose a danger if readily available to children are stored where they are inaccessible to children in care. Drinking water will be readily available to children both indoor and outdoor. LPA observed one functioning drinking water fountain in Play Yard A and one in Play Yard B.

LPA observed that there is a fence that enclosed the Play Yard (A and B) and Deck area. The outdoor activity space is equipped with a variety of age-appropriate toys and equipment such as a play structures. Areas around and under climbing equipment have artificial grass used as material that absorbs fall. Shade is provided by building overhang and canopies.

Director Natalia Makarova has obtained a Clearance for Criminal Background and Child Abuse Index Checks, She also meets the education and experience qualification requirement and had completed 16 hours of training on preventive health practices, which included pediatric cardiopulmonary resuscitation and pediatric first aid, and one hour on Child Care Nutrition. Her Pediatric CPR/1st Aid Certification expires 08/2021. Director has a current Child Development Site Supervisor Permit issued by the CA Commission on Teacher Credentialing that expires on 04/01/2024. Director has proof of immunity against Measles and Pertussis, and Health Screening with Tuberculosis clearance. Her Mandated Reporter Training for Child Care Provider Certification expires 08/03/2020.

LPA reminded the Director and Applicant of the applicable civil penalties for those adults who have not received fingerprint clearances, are not associated to the facility, and who come in contact with or provide care and supervision to the children. For an initial violation, civil penalty amounts to $100.00 per person per day up to $500.00 per person. For a subsequent violation within a 12-month period, civil penalty amounts to $100.00 per person up to $3000.00 per person.

This facility plans to provide Incidental Medical Services – IMS. A Plan of Operation that includes IMS has been submitted to the Department. LPA had also provided to facility the following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care centers and the ADA, available at: http://www.ada.gov/childqanda.htm.

Facility is encouraged to frequent the Department’s website at www.cdss.ca.gov [Shortcut: www.ccld.ca.gov] to access resources for Providers, Regulations, Annual License fee, Adoption of new Laws, etc.

Exit Interview was conducted, where this report was reviewed with Director and Applicant over a video conference call on 06/01/2020.

Director and Applicant agreed to make the following correction by 06/05/2020:

1. Per measurement, the facility only has enough indoor activity space for a maximum capacity of 35 children. Therefore, Applicant needs to submit an updated form LIC200A.

Applicant was informed that once Licensing Office receives the item above, Applicant's application for a Preschool Center License will be submitted for the final stage of review by Licensing Management.
