Applicant was informed that due to COVID-19 situation and "Shelter in Place" Order, this LIC809 Facility Evaluation Report will be emailed to the facility. Facility’s reply to the email will serve as acknowledgement that the report was received. Applicant Hearts and Hand Christian Childcare & Preschool is applying for a license to operate a Preschool Center at the address listed above. The requested capacity is 65 Preschool children ages 2 years to entry into first grade. A fire clearance was granted on 07/13/2020 by Gilroy Fire Dept. The Child Care Center is located on a functioning Church site: “Gilroy’s First Baptist church” LPA inspected the indoor and outdoor,on limit and off limit areas, as well as observed the Director and assistant measure the space available for the children. Indoor measurements are as follows: Preschool Classroom = (Total sq. ft.) – (Total Encumbered Space) Room 100: (736.79 sq. ft.) - (11.25 sq. ft.) = 725.54 sq. ft. Room 101: (385.22 sq. ft.) - (11.25 sq. ft.) = 373.97 sq. ft Room 102: (364.80 sq. ft.) - (11.25 sq. ft.) = 353.55 sq. ft. Room 103: (222.64 sq. ft.) – (0.00 sq. ft.) = 222.64 sq. ft. Room 106: (367.28 sq. ft.) – (11.25 sq.ft.) = 356.03 sq. ft. Room 107: ( 250.43 sq.ft.) – (0.00 sq.ft.) = 250.43 sq. ft. *Total Preschool Indoor Activity Space = (2282.16 sq. ft.) divided by (35 sq. ft. / child) = 65. Preschool children* There are 7 (105) sinks and 5 toilets (75) designated for Preschool children to use.

There is one staff restroom in common area that will also be used by children who become ill. Sick children will be isolated in the Director's Office and isolation equipment will be provided. A morning and afternoon snack will be provided for the children by the center. Children are required to bring their own lunch from home. The center as a kitchen with a preparation area where morning and afternoon snacks can be prepared. LPA observed trash cans with tight fitting covers for the disposal of solid waste. Drinking water indoors will be available via water jug and pitchers, fountains. LPA observed functioning smoke and carbon monoxide detectors, and fire extinguishers at the facility. The facility staff will clean toys and equipment daily with cleaning wipes and solutions that will be kept inaccessible to children. Medication will be stored in a secured cabinet inaccessible to children in office area. LPA observed toys, furniture, and equipment such as tables, chairs, mats, etc.

The Preschool playground is enclosed by fencing. Areas around and under climbing structure has material that absorbs fall. Shade is provided under Pergola canopy type setting. Drinking water outdoors is available via water pitcher, coolers and/or disposable cups. The center will not provide transportation. There is a working telephone which is ((408) 748-3500.
This facility plans to provide Incidental Medical Services – IMS. Facility had submitted a Plan of Operation that includes IMS to the Department. A review of staff records during today's visit indicates that Site Director Jessica Watts has received criminal background and child abuse index clearance and is associated to this facility. Director is advised of the applicable civil penalties for those adults who have not received fingerprint clearances, are not associated to the license and who come in contact with or provide care and supervision to the children. Penalty amounts: $100.00 per person per day, minimum of $100.00 to a maximum of $500.00 per person for an initial violation and a minimum of $100.00 to a maximum of $3000.00 per person for any subsequent violation within a 12-month period.

Exit interview was conducted, where this report was submitted via e-mail to the Site Director Jessica Watts

An exit interview was conducted prior to the conclusion of today's Tele-Pre-licensing inspection via FaceTime (video call). Director Jessica was advised that a license for 65 preschool children will be approved upon receipt of the following:

1) Submission of all required document

2) Applicant: Lannette R. Lozano background clearances has cleared (Fingerprints’)

3) Final Management's Approval
