On 11/01/2021 at 9:00am Licensing Program Analyst (LPA) Sabina Dodoo and Licensing Program Manager(LPM) Chandra Charles, met with licensee Vibhuti Patel and center Director Priya Sudharsan for an UNANNOUNCED ANNUAL REQUIRED INSPECTION. Present for the inspection were four teachers (S1, S2, S3, S4) who are all fingerprint cleared and associated, and 10 infants were present in the classroom. The classroom was within ratio. LPA provided licensee a copy of form LIC 126. The Child Care Center was toured to conduct a Health and Safety Inspection. The facility currently operates Monday through Friday from 8am to 5pm.
The child care center is a single story building with 6 classrooms(4 are preschool aged, 1 infant classroom and one music/dance classroom). Room number 7 (Penguin Classroom) is the infant classroom. LPA observed the classroom to be neat with proper ventilation and materials equipped to care for the infants. Children and parent(s) enter through the front door where there temperatures are checked. The parents sign in through a phone app called Bright Wheel. On Bright Wheel parents can access the activities their infants participated in, the food they ate and what time they took a nap.

The OFF-LIMIT AREAS are: the tenant space next to the building where the center is located.
The ON-LIMIT AREAS are the play yard located outside, and the infant classroom and the outside play area. The outdoor area has age appropriate toys for infants. The infant play area is separated from the rest of the pre-school aged children. There is a fence around the area and the ground is padded with a grassy green mat. In the classroom there are mats that children sit on, little cushion pillows to lay on, toys and stuff animals are present for infants to play with. There are 8 cribs located in a separate room with the infant names on them. There were mattresses that fitted the cribs and linens. The cribs were also see through. There was also a table called the toddler eating table with built in high chairs where one adult can sit and feed six children at a time. There is also a changing table which is an arms length away from a sink where the teachers change the diapers of the infants that is regularly disinfected with a baby pad inside. LPA observed infants sleeping on their backs and identified a sleeping log present where teachers documented observing infant who is asleep every 15 minutes. In terms of the food provided for the infants, the parents bring food from home which are stored in the refrigerator. The soiled linens for the bedding is sent home with parents every Friday to be washed. There are also extra linens stored in the infant cubbies with their names on it.

Every licensed child day care center shall have one or more carbon monoxide detectors in the facility that meet the standards established in Chapter 8 (commencing with Section 13260) of Part 2 of Division 12. The department shall account for the presence of these detectors during inspections.

All hazardous materials and toxins are kept out of reach from children and are not accessible. The Child Care Center has a fully charged fire extinguisher 3A-40-BC, working smoke detector, carbon monoxide, telephone and fully stocked first aid kit. Per licensee, there are no firearms on the premises. The children are able to wash their hands and paper towels are provided for them to wipe their hands. The furniture around the classroom is child proof and the toys are stored in safety bins that can be easily accessed by the infants. There was a Type B citation given to the facility due to carbon monoxide detector not being present at the time of inspection. This violation was corrected by the licensee while LPA was conducting inspection and it was successful.

The licensee completed the Health and Safety training, CPR/First Aid are all current . The licensee is in compliance with the immunization laws and has completed the mandated reporter training. The licensee conducts and documents fire and disaster drills every month and the last conducted drill was on 10/08/2021. All required forms are posted and visible for public review.

At 12:30pm LPA(Sabina Dodoo) reviewed 5 children’s files and 4 staff files,and documented on LIC 857 and LIC 859. There is a current roster available for review and LPA has requested for a copy to be emailed to Analyst. In the staff file a Technical Violation was given due to a staff member S4 missing the mandated reporter training certificate. The Staff interview was also conducted and documented.

Effective August 1, 2003 California Law requires Child Care licensees to report unusual incidents or injuries to children in care to child’s parents and to the Department of Social Services using the Unusual Incident/Injury form (LIC 624). Incidents must be reported within 24 hours to the regional office by phone and the written report, LIC 624 within 7 business days.

There are three deficiencies cited today. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the licensee Vibhuti Patel.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. Licensee was reminded that all adults 18 and over living or working in Child Care Center, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Licensee was reminded of the responsibility as a mandated reporter and the trainings must be done once every two years as well as CPR/First Aid needs to be renewed every two years and must be EMSA approved. LPA also encouraged licensee to frequently visit our website at www.ccld.ca.gov for licensing regulations and updates, as well as all forms can be downloaded.
