Licensing Program Analyst (LPA) Samantha Yip conducted an unannounced annual random inspection. LPA met with Site Director Jennifer Smith and Program Director Cathy Lusk and explained the reason for the inspection. Present during the inspection were 21 children and at least 7 staff present. The teacher child ratio was observed to be in compliance during today's inspection. Children were under the visual supervision of staff. Facility shares a building with TAM Infant center. Center uses Preschool 1 and Preschool 2.

License, Emergency Disaster Plan, and Notification of Parents Rights were observed to be posted. LPA reviewed sign in/sign out sheets, which contains full legal signature.

LPA toured in the inside and outside of the facility with Site Director Jennifer Smith. All disinfectant, cleaning supplies and other items that are dangerous to children were observed to stored inaccessible. LPA reminded Site Director that any disinfectant cleaning supplies and disinfectant wipes need to be stored where it is out of reach of children. LPA observed that there is sufficient amount of toys for children in care. Furniture and equipment, such as tables, chairs, and napping mats, were observed to be in good condition. The restroom for children's use was observed to be in functioning condition. Facility does have a waiver to share bathroom with TAM Infant Center. LPA observed that one of the toilets was leaking. Site Director stated that they have a work order in place to repair it. Storage containers for waste have tight fitted lids. LPA observed a fully charged fire extinguisher and smoke detector. Site Director stated that there are no weapons, such as firearms, stored on the premise.

The outdoor activity area is fenced. Facility does have waiver to share playground with TAM Infant Center. Play structure was observed to be in good condition. Areas around the play structure have resilient material to absorb falls. LPA discussed with Site Director about adding more resilient material and about ensuring all play equipment are in good condition, such as the trampoline and plastic play house. There were no bodies of water observed during today's inspection. Shaded rest area is provided through canopy.

Facility does provide meals and snacks to the children in care. Kitchen used to prepare meals and snacks was observed to be clean and sanitary. LPA reminded Site Director about disposing any food or milk that is expired. Drinking water is provided through water pitcher/cooler and cups.

Site Director stated that she currently does not have any children in care who requires Incidental Medical Services (IMS). 10 children's files were reviewed during today's inspection. The records reviewed include but not limited to admission agreement and immunization records.

7 staff files were reviewed during today's inspection. The records reviewed include but not limited education credit, Mandated Reporter Training, Health Screening, and immunization records to measles and pertussis. LPA reminded Director that Mandated Reporter Training requires renewal every two (2) years. There is currently at least one staff on site with a valid CPR/1st Aid, which expires on 06/2020. LPA observed that S-1, S-2, S-3, S-4, and S-5 did not have LIC 503: Health Screening in file. Director stated that she will send a copy of health screening for the staff in two weeks. LPA also reminded Director that immunization records for measles and pertussis

All staff present during today's inspection have cleared criminal record clearance, child abuse index, or exemption. LPA also reminded Director of the applicable civil penalties for those adults who have not received fingerprint clearances, are not associated to the license and who come in contact with or provide care and supervision to the children. Penalty amounts: $100.00 per person per day, minimum of $100.00 to a maximum of $500.00 per person for an initial violation and a minimum of $100.00 to a maximum of $3000.00 per person for any subsequent violation within a 12 month period.

In the areas evaluated during today's inspection, a Type B deficiency has been cited. An exit interview, where this report, the citation, plan of correction, and appeals rights were discussed and provided to Program Director Cathy Lusk. A notice of site visit has been issued and must be posted for 30 consecutive days.
