Licensing Program Analyst (LPA) Zaid Hakim conducted an Unannounced Annual / Random Inspection at the facility today. Upon arrival, LPA observed eight (8) infants and at least three (3) staff engaging in daily activities and met with Mr. James Dawson, Director. LPA toured the facility inside and out and observed required postings including but not limited to; the License to operate, Emergency disaster plan (LIC 610) and Notification of Parents Rights (PUB 393). Days and hours of operation are Monday through Friday 7:00am to 6:00pm. The facility is licensed to serve a maximum of twenty two (22) infants up to 2 years of age. During the inspection, LPA observed the facility to be in compliance with staff supervision and ratio requirements and all infants were attended to.

LPA observed that the facility is operating within the terms of the license and is in compliance with the approved fire clearance capacity. The facility was toured inside and out was observed to be clean, safe, sanitary, and in good repair. Disinfectants, cleaning solutions, and all other items that are dangerous to infants were observed to be securely stored and inaccessible. Furniture and equipment was observed to be in good condition, free of sharp, loose, or pointed parts. The facility has age appropriate furniture and equipment, including cribs, mats, feeding chairs, and changing tables. Feeding chairs for infants were observed to be clean and operable. Infant cribs were observed to be in good condition, without any extra items in the cribs, and equipped with wheels. The Director understands that infants shall only be in high chairs during feeding times or during feeding preparation times. Bottles brought from home were observed to be properly stored at the appropriate temperature and were labeled with the infant's name and dated. Storage containers for waste have tight fitting covers, changing tables are within arms length of a sink, and floors and carpeting are clean and free from obstructions. The Director understands that changing tables shall be disinfected in between diaper changes. Food storage areas in the classrooms were clean. Changing tables were clean and in good condition. The Director understands that when infants are sleeping in a crib, there shall be at least one staff on that side of the partition wall separating the crib area of the classroom or in close proximity. LPA and the Director discussed safe sleep best practices in detail.

No baby walkers or bouncers were witnessed during the inspection. The playground area used by the infant program is physically separate from play yards used by older age groups enrolled in a separately licensed program and the facility has an outdoor play yard schedule in place. The outdoor area is surrounded by secured fencing and the outdoor surfaces are safe for the children. LPA observed that a sign in/out sheet is used in each classroom that is updated throughout the day and signed by parents / authorized representatives with full signatures. The facility maintains an updated roster of children enrolled in the program.

No weapons are stored on site and no bodies of water are on the premises. LPA observed fire extinguishers, smoke detectors, carbon monoxide detectors, a central alarm system, and complete first aid kits throughout the facility. Emergency disaster drills are performed as required, last of which was done on 04/2019. The last safety inspection conducted by the local fire authority was in 07/2018 in which detectors, alarm systems, and fire extinguishers were serviced. LPA reminded the Director that all fire safety items and equipment should be maintained at least every 12 months. LPA reviewed a random sample of children's records and observed that there is a separate, complete, and current record for each infant enrolled in the center. LPA observed that each infant record reviewed included but not limited to; immunization records, needs and services plans, individual feeding plans, and emergency contact information. LPA observed that infant needs and service plans are updated at least quarterly, as required and signed and dated by facility staff and parents / responsible parties.

A random sample of administration and staff records were reviewed and all teachers and staff maintain valid background clearances. LPA reviewed a random sample of teacher files and observed that all teachers that provide care for infants have health screening forms and at least two staff on site maintains valid CPR / First Aid Certification which expires in 06/2019 and 01/2021. LPA observed that infant teachers meet the educational post secondary unit requirements, in the specified subject areas relating to infants. The facility does not provide transportation or meals to the infants. LPA and the Director discussed emergency disaster and preparedness, children's personal rights including no corporal punishment, forms of discipline, AB 633, healthy beverages in child care, safe sleep, and recent updates to the child seat belt law.

A review of staff records today indicate that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. LPA also discussed and reviewed immunization including Measles, Pertussis, and Influenza. LPA also reminded the Director of the applicable civil penalties for those adults who have not received fingerprint clearances, are not associated to the license and who come in contact with or provide care and supervision to the children. Penalty amounts: $100.00 per person per day, minimum of $100.00 to a maximum of $500.00 per person for an initial violation and a minimum of $100.00 to a maximum of $3000.00 per person for any subsequent violation within a 12 month period.

A Notice of Site Visit has been issued and must remain posted for 30 consecutive days.

No Deficiencies have been cited as a result of today's inspection.
