Licensing Program Analysts (LPA) Zaid Hakim conducted an Unannounced Annual / Random Inspection at the facility today. Upon arrival, LPA observed eleven (11) infants and toddlers and at least four (4) staff engaging in daily activities and met with Ms. Margaret Focha-Smart, Director. LPA toured the facility inside and out and observed required postings including but not limited to; the License to Operate, Emergency disaster plan (LIC 610) and Notification of Parents Rights (PUB 393). Days and hours of operation are Monday through Friday 7:00am to 6:00pm. The facility is licensed to serve a maximum of 13 infants ages seven (7) to thirty (30) months old with a toddler option for up to ten (10). During the inspection, LPA observed the facility to be in compliance with staff supervision and ratio requirements and that all infants were attended to.

LPA observed that the facility is operating within the terms of the license and is in compliance with the approved fire clearance capacity. The facility was toured inside and out was observed to be clean, safe, sanitary, and in good repair. Disinfectants, cleaning solutions, and all other items that are dangerous to infants were observed to be securely stored and inaccessible. Furniture and equipment was observed to be safe, in good condition, and free of sharp, loose, or pointed parts. The facility has age appropriate furniture and equipment, including infant cribs, cots, and changing tables within arms reach of a sink. Classroom furniture and equipment for infants were observed to be clean and operable. Infant cots and cribs were observed to be in good condition and cribs were observed without any extra items in them and covered with tight fitting linens. The Director understands that cots are to be stored without linens touching and reminded the Director that during nap time cots should be arranged in the classroom allowing at least enough space for a walkway.

Bottles brought from home were observed to be properly stored at the appropriate temperature. The Director understands that bottles for infants brought from home are to be labeled with the infant's name and dated on a daily basis. The Director stated infant bottles are brought to the facility ready to consume or commercially prepared formula is mixed by staff. Bottles are rinsed at the facility and are sent home and disinfected at home. Storage containers for waste have tight fitting covers, changing tables are within arms length of a sink, and floors and carpeting are clean and free from obstructions. The Director understands that changing tables or any equipment used shall be disinfected in between diaper changes (should it be needed). Food storage areas in the classrooms were clean. Changing tables were clean and in good condition. The Director understands that when infants are sleeping in a crib or infant cot, there shall be at least one staff on that side of the partition wall or in the same room without a partition. LPA and the Director discussed safe sleep best practices in detail and LPA shared additional information on safe sleep environments. No baby walkers or bouncers were observed during the inspection. The outdoor area is surrounded by secured fencing and the outdoor surfaces are safe and age appropriate for infants. LPA observed that an written sign in/out sheet is used that is updated throughout the day and signed by parents / authorized representatives with full signatures. The facility maintains an updated roster of infants enrolled.

No weapons are stored on site and no bodies of water are on the premises. LPA observed fire extinguishers, smoke detectors, carbon monoxide detectors, a central alarm system, and complete first aid kits throughout the facility. Emergency disaster drills are performed as required, last of which was done in 08/2019. LPA reminded the Director that all fire safety items and equipment should be maintained at least every 12 months. LPA reviewed a random sample of infant's records and observed that there is a separate, complete, and current record for each infant enrolled in the center. LPA observed that each infant record reviewed included but not limited to; immunization records, needs and services plans, individual feeding plans, and emergency contact information. LPA observed that infant needs and service plans are updated at least quarterly as required.

A random sample of administration and staff records were reviewed and all teachers and staff maintain valid background clearances. LPA reviewed a random sample of teacher files and observed that all teachers that provide care for infants have health screening forms and at least two staff on site maintains valid CPR / First Aid Certification which expires in 12/2020. LPA observed that infant teachers meet the educational post secondary unit requirements, in the specified subject areas relating to infants. The facility does not provide transportation. The facility does include off site field trips which are walks around the neighborhood. LPA and the Director discussed emergency disaster drills and preparedness, children's personal rights including no corporal punishment, forms of discipline, AB 633, healthy beverages in child care, safe sleep, and recent updates to the child seat belt law.

A review of staff records today indicate that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. LPA also discussed and reviewed immunization including Measles, Pertussis, and Influenza. LPA also reminded Licensee of the applicable civil penalties for those adults who have not received fingerprint clearances, are not associated to the license and who come in contact with or provide care and supervision to the children. Penalty amounts: $100.00 per person per day, minimum of $100.00 to a maximum of $500.00 per person for an initial violation and a minimum of $100.00 to a maximum of $3000.00 per person for any subsequent violation within a 12 month period.

LPA reminded the Director of the immunization requirement regarding Senate Bill 792 (pertussis, measles, and flu vaccines) for ALL STAFF THAT WORK DIRECTLY WITH CHILDREN and the required "Mandated Reporter" training that all staff will be required to complete starting January 1, 2018 per AB 1207.

LPA and the Director also discussed applicable updates to this training regarding length and material and the renewal period of every 2 years. LPA provided additional written information on Mandated Reporter Training, Active Shooter Information, Effects of Lead in Water, and Safe Sleep for Infants.
