Olesya Kalinowska was informed that due to the COVID-19 situation and "Shelter in Place" Order, this LIC809 Facility Evaluation Report will be generated at the Licensing Office and emailed to the facility. Facility’s reply to the email will serve as acknowledgement that the report was received. Applicant/ Representative is applying for a Change of Ownership to operate a Pre-school Center with a capacity of 39 at 26 Florido Avenue La Selva Beach, Santa Cruz CA 95076. A Fire Clearance has been granted on 07/2/2020. The Center will operate Monday through Friday from 07:00AM to 05:30PM, serving children 2-5 yrs old . Facility's phone # (831) 688-7790 Rooms 1,2, 3, Jr. Kindergarten/Big Room and Play Yard were inspected and measured to determine the maximum capacity that meets the requirement of 35 square feet of indoor activity space and 75 square feet of outdoor activity space per child. The measurements were observed agreed upon and taken by applicant's representative Olesya Kalinowska. LPA observed the process and provided direction over the video call.

Preschool has 3 (45) toilets and 3(45) sinks for preschool children. LPA observed hand soap and disposable paper towels in the restroom only cold water available in children’s sinks. The isolation area for children who are ill is the circle time area, isolation equipment (cots, chairs etc.) is provided for children to rest. There are cubbies to store their belongings. LPA observed age appropriate tables and chairs and play equipment. first aid supplies are stored in an off limits upper kitchen cabinet. Cleaning supplies are stored in an off limit upper kitchen cabinet

Facility will provide hot Lunch, and morning and afternoon snacks. Meals and snacks will be stored and prepared in the Kitchen area . The Kitchen is equipped with a refrigerator, microwave, oven, and stove to store, heat, and/or cool the food as needed. There are trash cans with tight fitted lids, and paper towels and hand soap near the sink.

The play yard is enclosed with fencing, No observed bodies of water. The outdoor activity space is equipped with age-appropriate toys and equipment such as a play structure and sand box. Areas around and under climbing equipment has tanbark used as material that absorbs fall. Shade is provided by trees and canopy. Drinking water is arranged to be readily available to children and there is a water fountain located out doors.

Facility staff will clean and disinfect toys and equipment daily with wipes and solutions. Detergents, cleaning compounds, sharps, medicines, and other items which could pose a danger if readily available to children are stored where they are inaccessible to children.

Olesya Kalinowska and Director (Hollis Delancey) have obtained a Clearance for Criminal Background and Child Abuse Index Checks. The director also meets the education and experience qualification requirement to be the Director of a Pre=School Center. Her CPR/1st Aid Certification expires on 12/07/21. Director has proof of immunity against Pertussis and Health Screening with Tuberculosis clearance.

LPA reminded the Olesya Kalinowska of the applicable civil penalties for those adults who have not received fingerprint clearances, are not associated to the facility, and who come in contact with or provide care and supervision to the children. For an initial violation, civil penalty amounts to $100.00 per person per day up to $500.00 per person. For a subsequent violation within a 12-month period, civil penalty amounts to $100.00 per person up to $3000.00 per person.

This facility plans to provide Incidental Medical Services – IMS. A Plan of Operation that includes IMS has been submitted to the Department.
