An inspection was conducted the facility by Licensing Program Analyst (LPA Snow) who met with Teacher Francis Clark. The facility file was reviewed prior to this visit. The staff of this program have criminal record and health clearances as a condition of employment. This is a Title 5 funded program located on the Buckeye Elementary School campus in room 5. The facility operates two half-day programs (separate morning and afternoon sessions), Monday-Friday. This facility operates the morning session from 8AM to 11:30AM.

The facility was toured inside and outside, and the floor and yard plan were verified. The items which could pose a danger to children (detergents, cleaning compounds and medications) were inaccessible to children. There were no poisons observed in the facility; they are locked in the janitors’ closet. The facility was free of flies, insects and rodents. The toys, floors, desks and other equipment and surfaces appeared clean, toxic free, safe and in good condition. There is uncontaminated drinking water available to children both indoors (drinking fountain) and outdoors (jug and cups). The children's bathrooms appeared in safe and sanitary condition. A current menu was posted. Food is prepared in the school's cafeteria. Garbage cans containing solid waste have tight fitting lids. The playground was free of hazards. The playground equipment and surface areas appeared in safe condition. There is wood chip cushioning underneath climbing structures and/or play equipment enough to absorb falls.

There were no bodies of water observed. The playground is shared with the Buckeye School of Arts Children's Center on separate schedules. The Teacher, Ms. Clark stated no weapons are stored on site and none were observed. During today's inspection, staffing ratios were being met and children were being properly supervised with two teachers and eight children. The facility was operating within the licensed capacity. At least one staff member (S#1) present possessed current CPR and First Aid certifications. This facility is providing Incidental Medical Services – IMS. LPAs reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. This report was reviewed and discussed with Teacher Ms. Clark. All licensing reports are public information and must be made available upon request for at least three years. There were no Title 22 deficiencies cited during today's inspection.

Children’s records & Staff records were on the computer and could not be accessed/located during the visit; the LPA will have to return.
