An annual inspection was made to the facility by Licensing Program Analyst, Wisehart. The facility file was reviewed prior to this inspection. This is a Title 5, Public Agency program licensed in rooms #26, #27 and #28 on the Meadow Lane Elementary School campus. Normal operating hours for the morning program are Monday-Friday, 7:45 am - 11:30 am in rooms #26, #27 and #28. Afternoon program hours are Monday-Friday, 11:45 am-5:30 pm in room #27. The facility follows the school calendar.

The facility was toured inside and outside and the floor and yard plan submitted by the licensee were verified. The items which could pose a danger to children (such as detergents, cleaning compounds and medications) were observed to be inaccessible to children. Poisons are locked in main campus janitorial closest. The facility was free of flies, insects and rodents. The toys, floors, desks and other equipment and surfaces were clean, toxic free, safe and in good condition. There is uncontaminated drinking water available to children both indoors and outdoors via drinking fountains. The children's bathrooms appeared in safe and sanitary condition. Room #27 has it's own bathrooms. There is a waiver on file for children in rooms #26 and #28 to use the schools bathrooms; the conditions of the waiver were being met. A current menu was posted in each room. Food prep areas are clean. Food is properly stored and free of contamination. Garbage cans containing solid waste have tight fitting lids. The playground was free of hazards. The playground equipment and surface areas were in safe condition. There is recycled rubber chip and wood chip cushioning underneath climbing structures and/or play equipment to absorb falls. There were no bodies of water observed. A school wide disaster was completed on 11/6/19.

Director stated no weapons are stored on site and none were observed. During today's inspection, staffing ratios were being met and there were 51 children were being supervised by 9 teachers/aides. The facility was operating within the licensed capacity. At least one staff member present during the visit (S1 Class 26; S2 Class 27 and S3 Class 28) possessed current CPR and First Aid certifications (Class #26 expires on 11/3/20 /Class #27 expires 10/13/20 and class #28 expires on 3/30/20). Thirty (10 in each class) children’s records were reviewed at 8:30 am(class #27)/ 8:50 am (class #28) and 9:20 am (class #26) and contained identification forms with authorized representative information, as well as medical assessments. Health Screening are a requirement of employment. The sign in/out procedure was reviewed and in compliance. This facility is providing Incidental Medical Services (IMS) to children. The Department’s IMS policy was discussed with the staff. (LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records.) This report, as well as the AAP Guide to Safe Sleep Practices brochure, were reviewed and discussed with the Director. All licensing reports are public information and must be made available upon request for at least three years.

Notice of Site Visit shall be posted for 30 days from today's visit.

There were no Title 22 deficiencies cited during today's inspection.
