25 | A case management inspection was made to this facility by Licensing Program Analyst (LPA) N. Cunningham. LPA Cunningham met with acting director, Makyla Waugh regarding an incident that occurred on 10/26/21. The facility had self reported an incident within the required time frame. On 10/26/21, an aide was leaving the toddler classroom and closed the door. When the aide closed the door, Child 1's finger got caught in the door jam. Staff immediately provided first aid for the child and contacted their parents. The child's parents picked up Child 1 and took them to the emergency room where they received butterfly stitches on their finger. It was corroborated that the staff member closed the door without realizing the child's finger was in the door jam. During today's visit, LPA Cunningham reviewed and photographed the door jam that Child 1 got their finger caught in. LPA observed a plastic accordion shield over the door jam which the acting director stated was placed there to prevent future incidents.
There were no Title 22 deficiencies cited during today's inspection. All licensing reports are public information and must be made available upon request for at least three years. This report was read and discussed with Acting Director, Makyla Waugh.
