LPA Laird interviewed two children (C1-C2) and two witnesses (W1-W2) on 12/20/23. LPA Cunningham conducted interviews with four parents (P1-P4) on 1/29 and 1/30/24. Two children stated the licensee places children on time out in the laundry room or garage for a long time. Child 2 stated the licensee hurts them and motioned how the licensee handles them. LPA Laird observed the two children appear fearful of the laundry room.
Based on the evidence obtained, the preponderance of evidence standard has been met, therefore the above allegations are found to be substantiated. California Code of Regulations, Title 22 is cited on the attached LIC 9099D. Exit interview was conducted, notice of site visit posted, and appeal rights were provided.
LPA N. Cunningham informed licensee, Mindy Riddle, that this report dated 2/29/24 documents 2 Type A citations which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.
Also, LPA N. Cunningham informed the licensee to provide a copy of this licensing report dated 2/29/24 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
An exit interview was conducted with licensee, Mindy Riddle. Appeal rights were provided. Notice of Site visit shall remain posted for 30 days.
