The facility pre-licensing inspection was conducted via tele-inspection due to the current state of emergency regarding the COVID-19 outbreak, by Licensing Program Analyst (LPA), Wisehart. The applicant has applied for a capacity of 70 school age children. The facility was toured inside and outside. The center will operate from Monday 12:30 pm - 6 pm and Tuesday - Friday 2:15 pm - 6 pm. The facility consists of two classrooms (North and South Activity Centers) serving children from Kindergarten to 12 years of age. The fire clearance was approved (8/10/20) for 70 children. The license is approved for 50 children based on indoor measurements and the outdoor area has ample space for the children. Required postings (Parents Personal Rights, Emergency Disaster Plan, Earthquake Preparedness Checklist, Menu, etc.) listed on Form LIC 311A will be posted. The applicant was advised that all staff are required to have a criminal background clearance on file with Community Care Licensing. The applicant is aware of the immediate $100 per day civil penalty for anyone providing care or supervision without a criminal record clearance.
The Director qualifications were reviewed and a waiver was granted on 10/7/20. Children will use the fenced outdoor play area. Sign-In/Out procedure were reviewed. The applicant/director understands that if a manual Sign-In/Out procedure is used; full legal signatures are required. There is a pull fire alarm and charged fire extinguisher in the facility, rated at least the 2A10BC. The facility will install a working carbon dioxide detector. All Exits are marked. There are two bathroom(s) with 4 toilets and 1 urinal and 4 sinks available for the children. There is one staff bathroom, which is separate. Though there are drinking fountains on site, due to COVID-19 water bottles are being used in lieu of the fountains at this time. The isolation area for a sick child will be established in the office area, a mat will be purchased for this purpose. Snacks are provided twice a day. Facility will provide a trash can lid for trash can(s) that would hold solid waste.

The food is properly stored, and refrigeration is provided. Age appropriate toys and activities are available for children. There is wood chip cushion material underneath the play structures sufficient to absorb falls. There are trees present to provide shade in the play yard and two water mister areas are available to children. The outdoor patio is covered. There are no bodies of water located on the property and none are to be added without prior notification and approval of the licensing agency. Incidental Medical Services (IMS) policy was discussed. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. This report, as well as the Guide to Safe Sleep Practices brochure and Lead Exposure Testing Flyer, were reviewed and discussed with the applicant/director. All licensing reports are public information and must be made available upon request for at least three years.
