An annual inspection was made to the facility by Licensing Program Analyst (LPA), Elpidia Hernandez Torres. The facility file was reviewed prior to this inspection. A review of the personnel report on 06/14/2022 indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

The facility’s operating hours are 06:30AM- 06:00PM, Monday – Friday. The facility was toured inside and outside and the floor and yard plan submitted by the licensee were verified. The items which could pose a danger to children (such as detergents, cleaning compounds and medications) were observed to be inaccessible to children. Poisons are not stored on premises. The facility was free of flies, insects and rodents. The toys, floors, desks and other equipment and surfaces were clean, toxic free, safe and in good condition. There is a blue couch in the "Tods" room that has a rip in the center, and the ceiling has a panel which has been pushed in exposing the air duct an advisory note was issued. There is uncontaminated drinking water available to children both indoors and outdoors in their own sippy cups on a tray. The children’s bathrooms were in safe and sanitary condition. A current menu was posted outside the "Tods" classroom. Food prep areas are clean. Food is properly stored and free of contamination. Garbage cans containing solid waste have tight fitting lids. The playground was free of hazards. The playground equipment and surface areas were in safe condition. There is soft cushioning underneath climbing structures and/or play equipment to absorb falls. There were no bodies of water observed. The assistant center director stated no weapons are stored on site and none were observed. During today's inspection, staffing ratios were being met and there were 21 infants being supervised by seven teachers/aides. The facility was operating within the licensed capacity. At least one staff member present during the visit (S1) possessed current CPR and First Aid certifications. 10 children’s records were reviewed at 11:40AM, and contained identification forms with authorized representative information, Four infants were missing LIC 701 Physician's report a deficiency was issued. Seven out of Ten infants were missing one dose of an immunization an advisory note was issued. All infants had a safe sleep log, some staff were not documenting per regulation, an advisory note was issued. Continued on 809-C

The "tods" room was observed using Cots as the nap equipment for infants a deficiency was cited. Seven staff records were reviewed at 2:02PM, and three staff were missing LIC 503 an advisory note was issued. The sign in/out procedure was reviewed and in compliance.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Assistant Center Director Shirin .

Based on observation, and interview, The "tods" room which is under the Infant license and provides services for infants 24 months and under was observed not using Cribs or portable cribs for infants while napping. LPA Hernandez Torres observed staff putting cots away. LPA asked staff in the "Tods" room the age of the infants and where they sleep. Staff replied the youngest in the room at the time was 11 months old and slept on a cot. LPA provided technical assitance and reviewed Safe Sleep Regulation with lead teacher in "tods" room and " infant" room as well as assistant center director. The Center did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Assistant Center Directer stated she nor the infant staff were aware the regulation was required for the "Tods" classroom as well. Assistant Center directer asked if she could have 30 days to purchase more Cribs/ Portable cribs and submit a reciept of purchase prior to that. LPA Hernandez Torres agreed a reciept as proof of purchase, and LIC 9098 Proof of Corrections Self Certification will be emailed/mailed or faxed to LPA Hernandez Torres on or before 07/14/2022. Assistant center director reached out to guardians and gave them LIC 701 to be completed by physician and place in the infant's file. Assistant Center Director agrred to submit a document with the four guardians Signature stating they received the LIC 701 and will have the form submitted to center, and the parents signiture with a date the day they dropped off the form.

Failure to correct the cited deficiency(ies), on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment.
