On 12/19/2024 at 11:40AM, Licensing Program Analyst, Sebastian Phouthavong made an unannounced Case Management visit to the facility and met with Licensee, Alyson Billingsly today to verify operation at the facility, and to address the facility's forfeiture. Prior to visit, on 12/06/2024, Licensee notified the department on the closure.

During the visit, LPA observed no daycare children in care and toured all areas of where care was being provided. There was no evidence of operation and/or childcare being provided at the facility. Licensee stated the last day of operation was on 10/25/2023 and due to personal reason is forfeiting her license. Previously, on 12/11/2024 the department received a statement on the close from Licensee.

The approval the Closure for the Family Child Care Home have been met.
