During the course of a complaint investigation visit, Licensing Program Analyst (LPA), Melchisedeck Augustin conducted an unannounced Case Management (CM) visit and met with Center Director (CD), Jaclyn Smith to evaluate compliance with teacher-child ratio requirements. On 03/21/24 and 03/25/24, LPA interviewed parent (P3), staff (S2-S3), and adult (A1) which confirmed they saw the facility operated out of ratio. According to P3, every morning the facility experienced staffing shortage, and P3 saw between 20 to 30 children were combined in the Tulip classroom with two Aides.
