A case management visit to the facility was made by Licensing Program Analyst (LPA) Y. Yang. On 04/16/24, the facility's licensee, Stephanie Sisson notified the LPA in writing that former center director Kathleen Kade's last day working at the facility as the center director was on 04/19/24.

During today's visit, the LPA met with the facility's new center director, Teresa Briare and provided technical assistance and information regarding the change of director procedure, Title 22 regulations, and documents to be submitted to the Department.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the center director, Teresa Briare. There were no Title 22 deficiencies cited during today's inspection.
