During the inspection, LPAs toured the facility, requested and obtained a current facility roster. Interviews were conducted with Director 1 (D1), Staff 1 (S1), Staff 2 (S2), and Child 1 (C1) on 6/10/22 starting at 11:50 AM.

D1 and S1 stated during interviews that staff was aware that Child 3 (C3) ran out to the outside back yard during the transition of moving the children inside the center. S1 stated that when the children were lined up to go inside, C3 ran to the corner of the play yard. S1 stated that she was the person to get C3 at the corner of the yard and took S1 a couple of minutes to get C3 inside. D1 and S1 stated that the facility has enough staff. D1 stated that C2’s parent informed the facility that C2 sustained an Impetigo infection, while at the facility. D1 stated there were no other children reported with this contagious infection. D1 stated that they had not witnessed C2 or any child sustained bruises or scratches while in care, if this had occurred staff would have contacted the parent and made an incident report. D1 stated they are not aware of a child being hit by another child, specifically C2. D1 stated we use behavior and preventive discipline. We go over the classroom rules and to be kind to one another. D1 and S1 stated when talking to the day care children the teachers/staff use encouraging words, including please and thank you. D1, S1 and S2 stated the teachers/staff use verbal prompts, separation, redirection, positive reinforcement, modelled behavior(s), and encouraged the child(ren) to use his/her word to express among the methods as a tool to manage a child(ren) challenging behavior(s).

During the inspection tours of the facility, LPAs observed the Staff appropriately communicating with the children, conducting indoor playtime activities with the children; and staff supervising and promptly tending to the needs of the children in care.

Based on interviews conducted and records reviewed, although the allegations may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violations occurred, therefore the allegations are Unsubstantiated.

There were no Title 22 deficiencies cited based on the above findings. Exit interview conducted and report was reviewed with facility representative, Nadereh Ali. Appeal rights were provided.
