25 | Licensing Program Analyst (LPA) Amy Strother arrived at the facility for the purpose of thr Center returning to active status. On 05/07/20 the Department received notification the center was closed due to the COVID-19 pandemic. On 08/31/21 the Department received form LIC9211 Request for Inactive Child Care License Status beginning 09/01/21 through 06/02/2022. On 02/22/22 LPA Strother received an email from Center Director, Mark Airey requesting to end inactive status and return to active status. During today’s visit, LPA met with Center Director, Mark Airey (D1).
A review of staff records indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. D1 was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
During today’s visit the facility was toured and the facility sketch verified. Operating hours are 8:00am – 3:00pm Monday – Friday. One staff was providing care and supervision to 6 preschool children, operating within the licensed capacity and ratio requirements. At least one staff (D1), present during the inspection possessed current CPR and First Aid certifications, which expires on 01/2024. LPA verified that the facility has a current fire extinguisher; rated at least 2A10BC, functioning smoke alarm and carbon monoxide detector. LPA Strother reviewed form LIC311A Records to be maintained at the facility with D1.
A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
There were no Title 22 deficiencies cited during today's inspection.

License is ready to return to active status.
