32 | Staff 1, S2, S3, S5, S7 and A5 were unable to corroborate the allegation that staff failed to meet child’s needs. S1, S2, S3, S7 and A5 all stated that C1 missed school frequently, missing appointments with outside resources on multiple occasions. A5 stated that they were discharged from providing services to C1 due to the number of missed appointments by C1. Review of attendance records dated 2/2019, 3/2019, 7/2019 and 8/2019 show that C1 missed 42 days out of the 56 days scheduled. Interviews and review of records dated 07/09/19 show that C1’s parent agreed to pick up C1 at 12:00pm, review of sign-in/sign out records indicate that the agreed upon schedule was followed. During months 7/2019 and 8/2019 C1 was picked up between 11:55am and 12:30pm. S1, S2, S3, S5, and A1 were unable to corroborate the allegation that a daycare child was left unattended, all denying the allegation, stating that no daycare child has ever been left unattended while at the facility. In the interview with S1, S1 denied the allegation that staff failed to provide adequate food service, stating that she personally fed C1 breakfast the morning that C1 was found to have head lice. S1 stated that staff do lice checks weekly and often check children’s heads for lice while they are distracted with eating. S1 stated that the morning C1 had lice, C1 did not want to continue to eat and began to cry when C1 saw their parent walk into the room and was then removed from the feeding chair to be with their parent. Interviews with staff did not provide corroborating evidence to support the allegations that staff failed to provide adequate food service.
Based on interviews conducted and records reviewed, although the allegations may have happened or are valid, there is not a preponderance of evidence to prove that the alleged violations occurred, therefore the allegations are unsubstantiated.
This report was reviewed and discussed with Executive Director, Lisa Grocott. Facility representatives’ original signatures were not recorded on the report; however, Lisa Grocott was provided with a copy of the LIC9099 Complaint Investigation Report (CIR) and Appeal Rights in an email dated 01/11/21 and confirmation of Read Receipt of the CIR is on file. All licensing reports are public information and must be made available upon request for at least three years. There were no Title 22 deficiencies cited during today's inspection. |
