25 | The facility inspection was conducted via tele-inspection due to the Covid-19 pandemic. The department has suspended all field operations and the applicant has agreed to conduct the video conference with LPA, (Licensing Program Analyst) Glenn Ouye.
Licensing Program Analyst (LPA) Glenn Ouye conducted a tele-visit video with licensee Nicole Monachello. The purpose of the visit was a change of capacity of her combination center licenses. The infant license is changing from a capacity of 14 to 22 effective 6/19/2020. The licensee has added the toddler option to the infant license.
The licensee toured LPA Ouye through the interior and outdoor area of the preschool. The interior floor space of the infant program area is physically separated from the toddler program. The licensee is approved for 10 infants and 12 toddlers.
The licensee's has submitted a new waiver request for the infants and toddlers to use the outdoor play area on a rotation. The licensee may start the rotation immediately while the waiver is being processed. LPA will follow up and mail the waiver to the licensee.
The capacity of 10 infants and 12 toddlers for a total of 22 children under the infant license is approved and is effective as of 6/19/2020.
The licensee was advised that because LPA's are not able to do on-site visits that the read receipt received by the department will provide acknowledgment that the licensee has received the document. |
