On 03/04/24 Licensing Program Analyst (LPA) Strother received an email from Licensee Nicole Monachello (L1) stating that Storybook Village Preschool- Infant center will be closing, last day of operation will be 03/15/24. Included in the email from L1, was an attachment of the letter dated 02/08/24 given to families, informing them of the closure. Today, an unannounced visit was made to the facility by LPA Strother to confirm closure and to obtain the facility’s physical license. L1 provided LPA with the physical license, signing and dating the back of the license with a statement regarding date of closure. LPA Strother met with L1 and toured the facility.

LPA Strother confirmed that licensed child care is no longer being provided. LPA observed empty classrooms and boxes packed with educational materials. No children were present during today’s visit. LPA did not observe evidence that care was being provided at the facility.

A copy of this report, was provided to Licensee Nicole Monachello. A closure letter will be mailed to the mailing address on file. Effective, 03/15/24 facility #493009744 is no longer licensed and can no longer provide licensed care to children.

LPA Strother will process the closure with the effective closure date of 03/15/24.
