25 | On February 11, 2022, Licensing Program Analyst (LPA) Kari McWilliams conducted an unannounced case management inspection. LPA toured the facility and a census was taken. LPA met with facility director, Tyra Toney. The purpose of today's inspection was to discuss an incident that occurred on February 7, 2022. A complete file review was conducted prior to today's inspection.
An Unusual Incident Report was submitted to the Fresno Community Care Licensing Office regarding an incident that occurred on February 7, 2022 where Child #1 was sitting on a play structure and Child #2 ran by and bumped into Child #1 causing Child #1 to fall forward off of the structure onto his hands with his arms outstretched.
It is reported that Teacher #1, was next to Child #1; went over to assess Child #1. Teacher #1 reported that Child #1 got up on their own and went to go sit down, Teacher #1 went to get an ice pack for child and walked Child #1 to Director Toney to call parent, dad picked Child #1 up about 10 minutes later. It is reported that parent took Child # 1 to urgent care on Tuesday February 8, 2022 where it was determined that Child #1 broke their right wrist from the fall. Teacher #1 reported that there were 14 children in attendance on February 7, 2022 and there were two staff members.
During outside inspection LPA McWilliams observed adequate padding in the play structure area. Director Toney reported that the structure that Child #1 was playing on was removed on Tuesday February 8, 2022 to hinder any other child from getting hurt. Director Toney stated that the structure was a round drum that sat about 2 feet high off of the ground.
Per Chapter 1, Division 12, Title 22 of the California Regulations, there is no deficiency being cited.
An Exit interview was conducted with Director Toney. A notice of site visit was given and must remain posted for 30 days.
