25 | On 7/07/2023 Licensing Program Analyst (LPA) Julie Baptista and Licensing Program Manager (LPM) Cynthia Brannon conducted an unannounced Case Management inspection. The facility called in an unusual incident on 5/26/23 regarding a child who fell outside in the play yard and was injured. LPA and LPM toured the area were the incident occurred and took a census.
On 5/26/23 a preschool child (C1) was running on the play yard towards the climbing structure. There were three teachers nearby. The teacher called out to child to stop running several times. When child reached the climbing structure, C1 slipped on the second step and fell forward resulting in a cut on his forehead. Teacher #1 assessed the wound and took the child inside where they performed first aid to child. The parent was called and child was picked up. Parent took child to the doctor that day and stitches were received at the wound.
LPA interviewed staff and reviewed records. After conducting the case management inspection and interviewing teacher, it was determined that the incident was an isolated incident and not a result of lack of care and supervision. Staff took appropriate measures to address the child’s injury, following appropriate policies, regulations, and reporting requirements.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited.
Exit interview conducted and report was reviewed with the Acting Site Supervisor, Araceli Garcia.
This report shall be made available to the public upon request. A notice of site visit was given and must remain posted for 30 days. |
