25 | On this date, Licensing Program Analyst (LPA) Luisa Gavoutian conducted an unannounced case management inspection. LPA met with Teacher Renee Vaga and Education Program Specialist Yvette Madrigal and toured the facility. The purpose of today's inspection was to follow-up on an unusual incident/injury that occurred on October 3, 2019.
On October 3, 2019, at approximately 2:00 p.m., Child 1 was climbing the steps of the play structure when Child 1 slipped and fell, causing a small cut on the top of Child 1's nose. Child 1 was administered first aid and the bleeding was stopped. Staff contacted Child 1's parent and Program Director, Judy Huerta. Child 1's parent took Child 1 to the emergency room, where a butterfly adhesive strip was applied. Child 1 returned to care on October 10, 2019.
During today's inspection, LPA spoke to Teacher Megan Brandao, who witnessed the incident. Substitute Teacher Yesenia Covarrubias also witnessed the incident, but was not present during today's inspection. Teacher Megan described the incident as it was reported to Community Care Licensing (CCL). LPA inspected the outdoor play structure where the incident occurred and observed it was in good condition, with no loose or broken parts. Sufficient supervision was being provided at the time of the incident. Teacher Renee stated she is reminding all children to walk on and near the play structure. The injury appears to be an isolated incident and staff took appropriate measures to administer first aid and notify parent. The incident was reported to CCL on the same day, October 3, 2019, meeting CCL reporting requirements.
No deficiencies cited in the areas observed today.
