On 06/17/2021, Licensing Program Analyst (LPA) Luisa Gavoutian conducted an unannounced complaint visit to provide findings. LPA met with Director Heidi de la Motte and Assistant Director Jennifer Renteria, who accompanied LPA during tour of facility. LPA discussed the allegation and took a census. During the course of the investigation, LPA made observations and interviewed staff, reviewed sign-in/out sheets, and reviewed facility records. Interviews conducted with Staff 1, Staff 2, and Staff 3, and photographs submitted, revealed that safe sleep regulations were being violated at the facility. Both Staff 1 and Staff 2 stated that infants were left on the ground after falling asleep and not immediately moved to a crib. Photographs were submitted depicting an infant asleep on their stomach on the ground.

On 04/27/2021, LPA entered the infant classroom to conduct observation and upon entering observed one awake infant in a crib that had been rolled out into the main classroom area and two awake infants in the same classroom area – one infant was eating and the other infant finished eating and was getting out of the chair. Staff 3 was supervising all three infants and stated the infant in the crib was asleep in the crib and no other staff were available to supervise all three infants; the crib was moved out into the classroom to allow Staff 3 to supervise the sleeping infant and two awake infants. Staff 3 stated the infant had been asleep for approximately five minutes and had woken up shortly before LPA entered classroom. LPA observed a blanket draped on one side of crib and Staff 3 stated the blanket had been there when the infant was asleep.

All facility staff have completed the “How Safe Sleep Savvy Are You?” quiz offered through the National Institute of Children’s Health Quality website. Director submitted a list of staff who have completed the quiz and the dates of completion to LPA on 05/11/2021.

Based upon observations, and information gathered through interviews, the preponderance of evidence standard has been met, therefore the above allegation is found to be substantiated.

Per California Code of Regulations, Title 22, Division 12, Chapter 1, these deficiencies are being cited on the attached LIC 9099D.

An exit interview conducted with Director Heidi de la Motte. A copy of this report and Appeal Rights were provided and discussed with Director.
A Notice of Site Visit Form was posted to parent's board and must remain posted for 30 days.
