25 | On November 28, 2022 Licensing Program Analyst (LPA), C Brannon conducted an unannounced case management inspection. LPA C Brannon met with director, Jonyce O’Neill. The purpose of today's inspection is to conduct a Case Management Inspection regarding a lead exceedance referred to, as an Action Level Exceedance (ALE) of over 5.5 parts per billion (bbp) in the water at this facility. LPA took a census and toured facility.
Fresno Regional Office received notification of water lead exceedance at the facility resulting from a recent water lead sampling test that was conducted on August 27, 2022. Director notified Licensing by email on November 2, 2022. Per director, previous emails were sent an LPA who is no longer working for the Department. Based on records reviewed from vendor California Rural Water Association (CRWA), lab testing, Enthalpy Analytical and discussions with director, Jonyce O’Neill, it has been determined there is an exceedance to outlet C – Sink Faucet in Kitchen on the church side, on site of facility. LPA C Brannon inspected outlet, and it was observed to be a sink in the kitchen that was impacted with a test result of 7.0 ppb (ALE) of lead. Director, Jonyce O’Neill, has replaced the kitchen faucet and has taped off faucet until further testing can be conducted. Director, Jonyce O’Neill, has notified authorized representatives of this correction in English. Per director, Jonyce O’Neill, kitchen faucet, C, is only used during November for Thanksgiving feast.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, a Type B deficiency is being cited (see LIC809 D). Director, Jonyce O’Neill, was provided a copy of appeals rights. An exit interview was conducted with, Director, Jonyce O’Neill.
