25 | On 6/12/2025, Licensing Program Analyst (LPA) Xona Xayavong conducted an unannounced Case Management inspection. LPA met with Director Laura Marquez. A tour of the facility was conducted, and a census was taken.
During an unannounced inspection on 04/03/2025, LPA reviewed video footage from inside a classroom dated March 25, 2025. LPA observed video footage that showed Staff #1 (S1) interacting with children inappropriately. LPA observed S1 grabbing Child #1 (C1) by the wrist/forearm and forcing C1 to sit down. LPA also observed S1 lifting Child #2 (C2) under the armpits and placing C2 down while forcing them to sit in a “criss-cross” position.
Additionally, LPA interviewed Parent #1 (P1), who stated that during pick-up time on the playground, P1 observed S1 grabbing Child #3 (C3) by the wrist/forearm and redirecting C3 to the time-out area.
During today’s inspection, LPA informed Director Laura Marquez that, based on the video observation and parent interview, S1 interacted with children inappropriately, thereby violating the children’s personal rights. Based on this evidence, the following Type B deficiency is being cited on the attached LIC 809D.
Per the California Code of Regulations (CCR), Title 22, Division 12, Chapter 1, a Type B deficiency is being cited on the attached LIC 809D.
An exit interview was conducted with Director Laura Marquez. A copy of this report and the Appeal Rights were provided and discussed. A Notice of Site Visit form was posted on the parent board and must remain posted for 30 days.
