25 | On 6/21/2021 Licensing Program Analyst (LPA), Cynthia Brannon, and LPA, Roman Iglesias, conducted an unannounced inspection. Upon arriving at the facility, LPAs observed pre-school age children commingling with school age children. LPAs took census and concluded that six school age children and four pre-school children were being supervised by Licensee, Linda Wiggins. Two other pre-school children were in the backroom napping and being supervised by staff # 1. LPAs asked Ms. Wiggins the reason for children commingling. Ms. Wiggins indicated children were commingling due to staff #1 being in the backroom with pre-school children. At that point, Ms. Wiggins instructed staff #1 to wake the napping children and take the rest of the pre-school children to a separate room. Additionally, LPAs observed no toilet paper inside the restrooms that the children use. Staff #1 stated that there was no accessible toilet paper inside the restrooms, in that children clog up the toilets and water runs down to the floor. Staff #1 then proceeded to place toilet paper inside each restroom, making it accessible to children. Additionally, Ms. Wiggins indicated she has a volunteer that comes in the mornings from 6:30 am through 7:00 am. Ms. Wiggins reported the volunteer is left alone for approximately 30 minutes, while Ms. Wiggings comes in at 7:00 am. LPAs reviewed the volunteer's personnel file, which lacked required documentation including pediatric First Aid /CPR training.
Per Title 22, Type A deficiency was cited during today's visit. Each enrolled child shall have the Acknowledgement of Receipt signed by parent, in the child's file. Any newly enrolled child for the next twelve months shall have the signed form in their file. Parent(s) of enrolled and newly enrolled children are to be given a copy of today's inspection report. The inspection report shall be posted for 30 days.
Per Title 22, these deficiencies are cited during today's inspection. Appeal rights were provided to licensee/director during today's inspection.
Today's inspection report shall be made available to the public upon request. Notice of site visit was posted before LPAs left facility.
