25 | On 06/18/24, Licensing Program Analyst (LPA) Anita Tristan arrived at the facility to conduct an unannounced Case Management Inspection. LPA met with Site Supervisor, Juan Vasquez-Espitia. LPA toured the facility, and a census was taken. The purpose of today's inspection was regarding an unusual incident that was reported to the Fresno Childcare Regional Office on 06/07/2024. On 06/07/2024 Child #1 was playing outside running on the concrete tripped and fell resulting in an open laceration on her chin.
During today’s visit LPA inspected the outside area where the incident took place and observed that the concrete is flat and in good condition. LPA did not observe any visual tripping hazards in the area where the fall occurred. There have been no further issues. Child #1 has returned and is doing well.
LPA Tristan observed a crack in the concrete on a walking path toward the back of the play yard. LPA informed site supervisor best practice would be to watch for future wear and tear or to have it filled in before it becomes a tripping hazard.
This appears to be an isolated incident and staff took appropriate measures to address the child following appropriate policies, regulations, and reporting requirements.
Exit interview conducted and report was reviewed with Site Supervisor, Juan Vasquez-Espitia. Appeal rights were provided and discussed.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited.
A Notice of Site Visit is provided and required to be posted for 30 days.
