25 | On 11/18/2021, Licensing Program Analyst (LPA) Luisa Gavoutian, conducted an unannounced Annual Inspection. LPA met with Licensee/Director, Eunice Mendoza, who accompanied LPA on a tour of the facility, indoors and outdoors. Hours of operation are Monday – Friday, 6:00 a.m. – 6:00 p.m.
All children are under supervision, including visual supervision of a teacher at all times. There is a ratio of one teacher supervising no more than 14 children in attendance. There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication, and other hazardous items are made inaccessible. All poisons are kept in a locked storage area.
LPA observed the facility had a fly infestation. Licensee stated the infestation began yesterday and they may have come from outside. LPA observed the majority of the flies were in Licensee's office. However, flies were observed in all areas of the facility, including infant, preschool, and school age classrooms, and kitchen. This deficiency is cited on the preschool license (LIC# 503810107).
LPA observed large grain salt on the carpets around the school age classroom. LPA observed the same salt scattered on the floors throughout the facility in the infant and preschool classrooms, in Licensee's office, and other areas of the facility. Licensee stated last night Staff 1 was at the facility alone and heard a toy go off, which led her to believe the building was haunted. Staff 1 proceeded to put salt throughout the facility in order to rid the facility of the evil spirits. This deficiency is cited on the preschool license (LIC# 503810107). Licensee vacuumed the classrooms to clean the salt. (Continued on LIC 809-C) |
