NARRATIVE
1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	A pre-licensing inspection visit was conducted on this date by Licensing Program Analyst (LPA) Brannon, who met with Licensee, Meredith Lawrence. The center is located at corner of Olive Ave and E Minnesota Ave. The licensee is requesting the following capacity of 8 infants. This program will operate year round, Monday through Friday from 7:30 AM to 5:30 PM. Parents will provide meals and snacks for their infants. A kitchen to prepare meals is not located on facility. Ill infants will be isolated in the director’s office until picked up by their parent or legal guardian. Room measurements taken and reviewed with licensee, Meredith Lawrence. There is one infant classroom. The total infant square footage is 627 which will accommodate the requested capacity of 8 infants. Adequate storage space available for infant's belongings. Outdoor storage is not available for toys and equipment. Toys and equipment are age appropriate. In the nap room, there are 8 cribs. In the infant classroom, LPA observed a child size tent. LPA recommended for the child size tent to be removed. LPA discussed the requirement for visual observation of infants at all times. Safe sleep regulations were reviewed. Outdoor measurements taken on this date total 2185 square feet which will accommodate the requested capacity of 8 infants. Adequate shade is available in the outdoor activity area. Licensee is utilizing mature trees for shade. There are no climbing equipment in the outside play yard. This facility plans to provide Incidental Medical Services – IMS.
