13 | Licensing Program Analyst (LPA) Julie Baptista conducted an unannounced inspection to complete the complaint investigation that was received on 2/16/23. LPA was greeted by two assistants. LPA met with licensee, Sondra Louis. A tour of facility was conducted, and census was taken.
During the complaint investigation it was discovered that personal information about a day care child was disclosed. Interview findings revealed that licensee and staff #1 provided personal information of child#1 to an unrelated adult of child#1. Form LIC 700 Identification and Emergency Information Child Care Centers/Family Child Care Homes and form LIC 627 Consent For Emergency Medical Treatment states that the information is confidential. Child#1’s personal confidential information was disclosed to someone who does not work or attend the day care home.
After interviews, records review, and LPA observance this agency has investigated the above complaint allegations; the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED.
Per California Code of Regulations Title 22 Division 12 Chapter 1 the following deficiency is being cited today (See 9099-D). An exit interview was conducted with Sondra Louis, Licensee, and Appeal Rights were provided.
