On 2/1/2023 at 2:30pm, a Required - 1 Year inspection was made to the facility by Licensing Program Analyst (LPA), Laura Chavez. The school-age program operates during the traditional school year (Kinder:12:45pm-6:00pm),(School-age:3:15pm-6:00pm) Monday-Friday. During the spring, summer and winder breaks the program opertes 7:00am-6:00pm). The facility was toured at 11:15am inside and outside and the floor and yard plan submitted by the licensee were verified. The school-age program operates at the Live Oak Church of the Brethren.

During today's inspection there were no school-age children. There are no pools or bodies of water on the premises. The facility has a waiver to allow school age children and preschool age children to commingle between 7:00am-7:30am and 5:00pm-6:00pm, and the terms of the waiver are being met.

Five children's records were reviewed at 12:05pm. Seven staff records were reviewed at 3:15pm.

Facility Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. There were no deficiencies cited during today’s inspection.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
