25 | On 2/9/24 at 9:11am, an annual inspection was made to the facility by Licensing Program Analyst (LPA), E. Laird. This program is operated by public agency and a Title 5 funded program. Operating hours are 8:30am-11:30am for morning session and 12:15pm-3:15pm for afternoon session, Monday–Friday. The facility was toured at 9:15am inside and outside and the floor and yard plan submitted by the facility were verified. Facility operates in building K2.
The site supervisor, Emily Perez and three teachers were supervising 17 children, and operating within the licensed capacity and ratio requirements. This preschool class is an Inclusion class with additional teachers. There are no pools or bodies of water on the premises. The outdoor activity space was cushioned with rubber mats and free of hazards.
Five children's records were reviewed at 9:37am. Two staff records were reviewed at 10:33am.
The following deficiencies were cited: At 9:25am LPA E. Laird observed an epi-pen for C6 which had expired on March 2021. LPA E. Laird showed the expired medication to the site supervisor, Emily Perez at 9:26am and Fawn Ueberschaer, Director at 10:29am. (see LIC 809D):
LPA Laird informed Fawn Ueberschaer, director that this report dated 2/9/24 documents 1 Type A citation(s) which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.
Also, LPA E. Laird informed Fawn Ueberschaer, director to provide a copy of this licensing report dated 2/9/24 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
