On August 10, 2023 at 10:43 AM, Licensing Program Analyst (LPA) J. Helton conducted an unannounced complaint inspection and met with director LeeAnn DeLuze. It was alleged staff left child (C1) in soiled diaper resulting in diaper rash.

During the initial visit at 10:45 AM, the director was interviewed. The director stated that most children in the classroom are potty trained, and they are reminded to use the bathroom and wash their hands throughout the day. Some of the children are not completely potty trained and one child (C1) did go home with a wet diaper on one occasion. Director states she has spoken to staff (S1) regarding checking on children more frequently and before leaving the facility for the day. Director stated the application of diaper rash cream should be documented on activity logs through pro-care, however, sometimes they are not. The Director stated she has spoken to staff (S1) to make sure to log activities throughout the day. LPA received activity logs for a 2-week period of time. Activity logs could not be provided for all the children, or everyday requested. The activity logs are not being updated. C1 activity log was for 7/7/2023 and only provided sign in/out times.

It was alleged that staff did not provide basic care for daycare child (C1), specifically not ensuring meals are eaten. Staff did not apply sunscreen on C1 resulting in sunburn, and staff did not apply mosquito repellent resulting in multiple bites.

During the initial visit at 10:45 AM, the director was interviewed. Director stated the application of diaper rash creams, sunscreen and repellent should be documented on activity logs through pro-care, however, sometimes they are not. The Director stated she has spoken to staff (S1) to make sure to log activities throughout the day. LPA received activity logs for a 2-week period of time. Activity logs could not be provided for all the children, or everyday requested. The activity logs are not being updated. C1 activity log was for 7/7/2023 and 7/31/2023 and only provided sign in/out times.

During the initial visit and 11:00 AM, LPA interviewed S1. S1 stated that sunscreen and mosquito repellent is applied to children if authorized by parents. Parents provide sunscreen and repellent. S1 was asked about meals with children. S1 stated that she encourages children to eat healthier food which parents provide first; some children eat more than others. Any food that is not eaten is returned home with the child. Children bring breakfast, lunch and snack from home.

Between the dates of September 5th and September 11th, 2023, LPA attempted to interview 7 clients/parents (P1-P7). 4 were successful. P2-P4 and P6 had no concerns or issues with the care given to their children. P2-P4 and P6 had no concerns or issues with sunscreen or repellent practices at the facility. P3 and P6 stated they do not provide repellent for their child. P3 stated, in the past if there are a lot of mosquitos outside staff has called to obtain permission to use on child.

Although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred, and the findings are unsubstantiated.

Exit interview conducted and report was reviewed with the Director LeeAnn DeLuze.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

During the initial visit at 11:00 AM, LPA interviewed S1 who stated diaper rash cream is used and is not documented for parents.

Between the dates of September 5th and September 11th, 2023, LPA attempted to interview 7 client/parents (P1-P7). 4 were successful. P2-P4 and P6 had no issues with diaper care.

Based on the evidence obtained, the preponderance of evidence standard has been met, therefore the above allegation is found to be substantiated. California Code of Regulations, (Title 22), is being cited on the attached LIC 9099D.

Exit interview conducted and report was reviewed with the Director LeeAnn DeLuze.
