An annual inspection was made to the facility by Licensing Program Analyst, Wisehart. The facility file was reviewed prior to this visit. This is a Title 5 Program located in a temporary building behind the Bidwell Elementary School and is closed during the summer and school holidays.

Operating hours are 8:15 - 11:15 and 12:30pm - 3:30pm, Mon-Fri. The facility was toured inside and outside and the floor and yard plan were verified. The items which could pose a danger to children (detergents, cleaning compounds, and medications) were inaccessible to children. The Site Supervisor stated poisons are locked in the off-site school maintenance department. No poisons were observed. The toys, floors, desks and other equipment appeared clean and safe. There is drinking water available to children both indoors and outdoors. The children's bathrooms appeared in safe and sanitary condition. A current menu was posted on parent board near front door. Food is prepared at the elementary school and breakfast and lunch are provided. Food prep areas are clean. Food is properly stored. There is a working smoke detector, carbon monoxide detector and fire extinguisher in the facility. The playground is completely fenced and free of hazards. The playground equipment appeared in safe condition. There is wood chip cushioning underneath climbing structures and/or play equipment to absorb falls. There were no bodies of water observed.

The LPA reviewed 3 out of 3 staff records at 11:04 am and health screen forms were on file. Staff (S1) has a valid first aid (expires 8/20)/CPR card (expires on 6/20). The LPA reviewed 23 out of 23 children records at 11:19 am , and contained identification forms with authorized representative information, as well as medical assessments. The sign in/out procedure was reviewed and in compliance. The last disaster drill(s) were recorded on 10/23/19 for the AM class and 10/25/19 for the PM class. This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. During today's visit staffing ratios were being met. Staff had required Mandated Reporter certifications. The report was reviewed and discussed with the Site Supervisor. All licensing reports are public information and must be made available upon request.
Notice of Site Visit shall be posted for 30 days from today's visit.

There were no Title 22 deficiencies cited during today's visit.
