An inspection was conducted on the facility by Licensing Program Analysts (LPA), Emilia Grisak and Patricia Pacheco. The facility file was reviewed prior to this visit. A review of the personnel report on file indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

The facility’s operating hours are 7:00 am - 5:30 pm, Monday-Friday. The facility was toured inside and outside and the floor and yard plan submitted by the licensee were verified. The items which could pose a danger to children (such as detergents, cleaning compounds and medications) were observed to be inaccessible to children. Poisons are locked in a closet in the bathroom. The facility was free of flies, insects and rodents. The toys, floors, desks and other equipment and surfaces are clean, toxic free, safe and in good condition. There is uncontaminated drinking water available to children both indoors and outdoors via water faucet. The children's bathrooms are in safe and sanitary condition. A current menu was posted by the entrance. Food prep areas are clean. Food is properly stored and refrigerated as needed. Garbage cans containing solid waste have tight fitting lids. The playground was free of hazards. The playground equipment and surface areas are in safe condition. There is pea gravel cushioning underneath climbing structures and/or play equipment to absorb falls. There were no bodies of water observed. The director stated that there are no weapons are stored on site and none were observed. LPAs reviewed sign in/out sheets and observed that there were 22 children signed in but only 15 children in care. LPAs reviewed sign in/out records for the month of August 2019, and observed that there were several instances when children were not signed out as required. During today's inspection, staffing ratios were being met and 15 napping children were being supervised by 2 teachers. The facility was operating within the licensed capacity. The director agreed to review all sign in/out sheets daily and review policy with children's authorized representatives and staff. The director agreed to submit complete sign in/out records from 08/12/19 - 08/23/19 as evidence of correction.

was not met as evidenced by: per review of records, the staff failed to ensure that all children in care were signed out by their authorized representatives; there were several instances each day of the month. This posses a potential threat to the health and safety of children in care.

certifications. 8 children’s records were reviewed at 1:55 pm, and contained identification forms with authorized representative information. 4 staff records were reviewed at 2:00 pm, and contained documentation of education and training, as required. This facility is not currently providing Incidental Medical Services (IMS). The Department’s Incidental Medical Services (IMS) policy was discussed with the Licensee/Director.
