25 | On this date, Licensing Program Analyst (LPA) Kathy Pacheco conducted an unannounced case management inspection. LPA met with Lead Teacher, Maria Mendez, to discuss an unusual incident that was reported on 12/12/19. A complete file review was conducted prior to the inspection. LPA took a census and reviewed children and staff files.
LPA was unable to interview the teacher and child that were involved in the unusual incident because the child was no longer attending the facility and the teacher was not present due to illness.
LPA advised Lead Teacher, Maria Martinez, that LPA will conduct another inspection to the facility regarding the unusual incident.
Per California Code of Regulations Title 22, Division 12, no deficiency cited during today's inspection.
LIC 9213 NOTICE OF SITE VISIT FORM IS REQUIRED TO BE POSTED FOR 30 DAYS.
