25 | Licensing Program Analyst (LPA) Theresa Marquez conducted a Case Management inspection and met with Director Denise Egge. LPA Marquez took a census and toured the outside playground area.
The purpose of this visit is to discuss the self reported unusual incident that occurred on July 12, 2019. The Director reported that on July 12, 2019, 1 staff was supervising 6 children in the outside play area. Two children began to throw sand at one another and the Teacher attempted to redirect them. The other four children were inside a playhouse and began to argue. The Teacher called them to come outside of the playhouse and Child #1 began to cry and confessed to the teacher that the children had pulled their pants while inside the playhouse. The incident was not witnessed by staff. Child #1 stated the incident was initiated by Child #2.
On July 12, 2019, Director Egge notified all 4 parents in witting of this incident.
On July 17, 2019, Egge submitted a Suspected Child Abuse Report to Child Protective Services as well as notifying Community Care Licensing (CCL) Officer of the day via telephone and submitting an Unusual Incident report via CCL fax.
The 2 playhouses were disassembled and removed permanently and an additional staff was added during playground time. The facility has now implemented additional outdoor and indoor protocols. Child #2 along with Child #1, #3 and #4 are no longer enrolled at facility because all 4 children began Kindergarten in August.
LPA Marquez reminded Director all future Unusual incidents are to be reported to CCL within 24 hours.
Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, no deficiencies were cited. |
