25 | On 1/9/2023, Licensing Program Analyst, (LPA) Theresa Marquez conducted a Case Management inspection and met with Director Blair Vedenoff. LPA Marquez observed the condition of the preschool and took a census. The purpose of this inspection is in regards to a self reported incident that occurred at the preschool on 12/15/2022.
On 12/15/2022, during lunch break, 4 staff were supervising 43 children in the playground. A child exited the playground through a gate and walked onto the front facility parking lot unsupervised. Staff in the front office observed the unsupervised child and redirected the child back to the playground. Staff estimate that the child was unsupervised for under 3 minutes. The child's parent were immediately notified of the incident. The same day, the facility installed weatherproof gate/door alarms onto the 3 outgoing gates to prevent future elopements.
LPA Marquez informed Director Vedenoff that this report dated 1/9/2023 documents one Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.
Also, LPA Marquez informed the Director Vedenoff to provide a copy of this licensing report dated 1/9/2023, that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
An immediate $500 Zero Tolerance penalty is being assessed today.
Per Chapter, Division 12, Title 22 of the California Code of Regulations, the following deficiency is cited. See the attached LIC809-D. An exit interview was conducted and report was reviewed with Blair Vedenoff. A copy of Appeal Rights and the Notice of Site Visit were given and must remain posted for 30 days.
