Licensing Program Analyst (LPA) Theresa Marquez conducted a Case Management inspection and met with Site Supervisor Linda Coffman.

The purpose of this visit is to confirm the Confirmation of Removal of Raquel Perez.
Per Site Supervisor Coffman, Perez was never hired with Family Focus.

The Confirmation of Removal form CBCB-5.1 was completed and given to LPA Marquez during this inspection.

No deficiencies observed during this inspection.
