On 5/6/2022, Licensing Program Analyst, (LPA) Theresa Marquez conducted a Case Management inspection and met with Site Supervisor, Linda Coffman. LPA Marquez toured the preschool and took a census. The purpose of this inspection is in regards to a self reported incident that occurred at the preschool on 5/2/2022.

On 5/2/2022, at approximately 3:30 PM, thirteen children were outside with 4 staff in the play yard, when a child went inside the facility to a classroom by opening an unlocked door. There were no teachers in the classroom and the child was inside for approximately one minute without visual supervision. The child looked through a window and saw his parents car and ran out the classroom door to meet them. Teacher #1 was in the Butterfly classroom at the time of the incident and she was the first to be advised by the child's mother of the incident. The preschool has recorded video cameras in all classrooms and staff were able to determine that the child was without supervision in the classroom for about one minute before exiting.
During this inspection, Site Supervisor Coffman provided the following Action Plan effective 5/3/2022, to LPA Marquez;
New auditory alarms have been installed on all classroom exits to replace the old auditory alarms to ensure that all alarms are operational.
Parents will now enter only through the front reception area for pick up and drop off.
Parents will now sign in/sign out where the child is located whether it be on the play yard or in the classroom. Parents with their child will exit only through the reception area.
A staff person is now placed at Zone 1 while children are outside.

Per Chapter, Division 12, Title 22 of the California Code of Regulations, the following deficiency is found: A child entered the facility and was without visual supervision in a classroom before exiting. (see LIC809-D)

An exit interview was conducted and report was reviewed with Site Supervisor Linda Coffman. A copy of Appeal Rights and the Notice of Site Visit was given and must remain posted for 30 days. Continued on LIC809-C

LPA Theresa Marquez informed Linda Coffman that this report dated 5/6/2022, documents one (1) Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care.

LPA Marquez also informed Linda Coffman to provide a copy of this licensing report dated 5/6/2022, that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Failure to comply with posting requirements shall result in an immediate civil penalty of $100

RESPONSIBILITY FOR PROVIDING CARE & SUPERVISION- The licensee shall provide care and supervision as necessary to meet the children's needs.
(1) No child shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation.

All staff are to review the CCL video SUPERVISING CHILDREN IN CHILD CARE CENTERS. Director is to submit a signed staff roster as evidence that ALL staff have reviewed the CCL video by May 13, 2022.


An immediate $500 Zero Tolerance penalty is being assessed today.
