On 12/19/2025, at 10:00AM, Licensing Program Analyst (LPA) Cynthia Alvarez conducted an unannounced inspection at the abovementioned Facility to deliver the findings for a Complaint related to alleging Neglect/Lack of Supervision. LPA met with Rebecca Mendoza, Director of the facility and advised them of the purpose of the inspection. It should be noted LPA observed 30 children on site along with an 11 teachers (cleared and associated) providing care and supervision.

The investigation included observations, children’s records reviews, interviews and two unannounced site inspections. As noted above, the specific allegations of the complaint are that the facility left a child unattended and the incident was not reported to parents.

LPA was unable to corroborate or validate the allegations of the complaint. Contrary to the complaint, teachers at the facility did not note any instance of a child being left unattended. Further, the Director and Teachers ensure proper care, which includes constant supervision and a safe environment.

LPA reminded Director of active supervision and advised to always conduct head count of the children especially during transitions.

Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED.

A Notice of Site Visit (LIC 9213) and Appeal Rights (LIC 9058) are provided to Director. The Notice of Site Visit must remain posted for 30 days or a civil penalty of $100.00 may apply.

Exit interview conducted and report was reviewed with Director Rebecca Mendoza.
