On January 29, 2024 at 2:29 PM Licensing Program Analyst (LPA) Laura Villanueva conducted an unannounced inspection of the Catalyst Kids-Green Valley Child Care Center (CCC) to deliver the finding with respect to the allegations noted above. LPA met with Eudulia Lopez Lia, Master Teacher of the CCC and explained the nature and purpose of the investigation. LPA observed 34 children were present at the time of the inspection, along with 9 staff members.
The investigation included two site inspections, documents obtained and interviews with the Complainant, the Site Supervisor, staff, as well as the parents of children in care. As noted above, the specific allegations of the Complaint are regarding the staff does not ensure that day care child's diapering needs are being met while in care and Staff did not adequately supervise day care children in care.

The allegation was collaborated by parents and staff at the CCC, which included written declaration were provided to LPA by staff on October 27, 2023, detailing staffs diaper changing procedures, in attempting

On January 29, 2024 at 2:29 PM, Licensing Program Analyst (LPA) Laura Villanueva conducted an unannounced inspection of the Catalyst Kids-Green Valley Child Care Center (CCC) to deliver the finding with respect to the allegations noted above. LPA met with Eudulia Lopez Lia, Master Teacher of the CCC and explained the nature and purpose of the investigation. LPA observed 34 children were present at the time of the inspection, along with 9 staff members.
The investigation included two site inspections, documents obtained and interviews with the Complainant, the Site Supervisor, staff, as well as the parents of children in care. As noted above, the specific allegations of the Complaint are regarding the Staff did not adequately supervise day care children in care.
Interviews, documents obtained as well as LPA's observations did not corroborate the allegations of day care child sustained an unexplained injury.
Although the child had a streak of redness on the neck, the investigation was unable to corroborate the sustained the injury while in care. The reporting party stated the child did not require medical attention,

and was not taken to the hospital for medical treatment and was unable to provide a case number for the police report filed.
Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED.

A closing interview was conducted with the Master Teacher, Eudulia Lopez Lia. The Master Teacher was provided and advised of their right to appeal (LIC 9058). A copy of this report was reviewed and provided to the Master Teacher.

The Notice of Site Visit (LIC 9213) was also provided to the Site Supervisor as required by H&S Code Section 1596.817. The Notice of Site Visit must remain posted for 30 days or a civil penalty of $100.00 may apply.

- The infant shall be kept clean and dry at all times. Each infant's clothing and diapers shall be changed as often as necessary to ensure that the infant is clean and dry at all times.

Site Supervisor/Master Teacher is to provide a written plan of correction to CCLD on how the center will prevent future occurrence of Staff ensuring that day care child diapering needs are being met while in care, via email to: Martina.Jimenez@dss.ca.gov by 2/5/2023, by the close of business.

to complete the changing of a child’s diaper, which included, having several staff attempting and contacting parents to come into the center to complete the diaper changing of a child if need be.

During interviews with staff, and parents informed LPA the staff will attempt to change the child’s diaper, if the child refuses to have their diaper changed, the staff will request another staff member attempted to complete the changing of the child’s diaper. If the child continues to refuse to allow staff in completing the diaper to be changed the center will contact the parents to come into the center and change the child’s diaper or wait until pick up time and advised the parent the child has a soiled diaper, due to after several attempts made by staff to change the child’s diaper the child refused to have their diaper changed. The staff would then offer parent to come into the center and change child’s diaper.

Based on LPAs observation, record reviews and interviews, the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED. California Code of Regulation, (Title 22 Division 12 and 101223 (a)(1) and 101227 (a)(14), are being cited on the attached LIC 9099 D).
