25 | Licensing Program Analyst (LPA) Ana Tolentino made an unannounced CASE MANAGEMENT- INCIDENT inspection to the center. LPA Ana Tolentino met with Director Teresa Martinez and explained the purpose of the visit to follow-up on an incident that was self reported to the department on 4/3/19.
On April 2, 2019, school picture day was scheduled, as three teachers were moving a group of 13 children, three of the children were having challenges in the transition of the group picture. As teacher#1 was redirecting two children, Child#1 ran back inside the classroom and observed the children thru the window. Volunteer#1 (fingerprinted) was entering the classroom to deliver lunch noticed Child#1 sitting in the small couch observing the children thru the window. Volunteer#1 notify Site Supervisor immediately who attempted to have the child walk out for the group picture. Child#1 was refusing to go out and Staff#1 was called to support Child#1 to transition with the rest of the group. Site Supervisor contacted parents and informed them of the incident, parents advised Child#1 did not like taking pictures.
Based of interviews conducted with staff and LPA's review of records, Child#1 did not pose an immediate health or safety risk.
Based on staffs immediate response, the following Type C technical violation advisory was noted: 101229 Responsibility for Providing Care and Supervision. See the LIC 9102 for advisory cited.
FAILURE TO POST THE NOTICE OF SITE VISIT FOR 30 DAYS MAY RESULT IN A $100 CIVIL PENALTY. |
