Licensing Program Analysts (LPA's) Betzayra Cervantes and Michael Avila made an unannounced visit for the purpose of conducting a Case Management inspection. On 09/26/2019, the facility self-reported a child (C1) left the classroom without the teacher aware of his absence.

LPA's met with Site Supervisor Teresa Martinez and discussed the nature and purpose of the visit. LPA's and Site Supervisor together toured the facility inside and outside and conducted staff interviews.

During the transition at or around 8:50 AM, two teachers S1 and S2 were caring for 12 children in Room #4. The children were transitioning from the carpet to the tables, when teacher (S1) returned from her scheduled break. C1, pushed the classroom door open and walked outside and was crying out for his mother. He walked out to teachers S3 and S4, who were on the playground caring for 16 children at the time of the incident. S1 stated that she was not aware that C1 had walked outside. However, the child was never without supervision given another teacher (S3) observed the child leaving the classroom and escorted C1 back to teacher's (S1) group. Parents were notified of the incident. Site Supervisor advised that they will be hanging bells on the doors to help monitor whenever anyone leaves the classroom. Teachers have been advised that supervision must be provided at all times, however given the child was immediately discovered, LPA's determined there was no lack of care and supervision.

No deficiencies were issued during this facility visit.
