1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On February 8, 2024 Licensing Program Analyst's (LPA's) Susana Martinez and Aaliyah Zendejas conducted an unannounced required 3-Year inspection. LPA's met with site supervisor Ashley Rodriguez and advised her of the purpose for the inspection. Together with site supervisor, LPA's toured the facility inside and outside. At the time of inspection there were 32 children in the care of 6 adults. No bodies of water were observed during the inspection. The center is located on the Rios Rosales Elementary School campus and occupies rooms G-1 and G-2. The center operates Mon- Fri 8 AM- 2 PM and provides breakfast, lunch and afternoon snack which is provided by the school district. LPA's observed food menus posted in both rooms. The required licensing documents were observed to be posted at the entrance of each room. Each of the classrooms have age-appropriate toys and furniture readily accessible for children in care. LPA did not observe any hazards/toxins items accessible to children. First Aid kits were observed throughout the facility. Currently the facility uses an App called Child Plus for sign-in/sign-out. LPA's verified the number of children on the sign-in/sign-out log match the number of kids present. The outdoor playground is shaded and has an ample amount of space for children to play. LPA observed the playground has age-appropriate toys and structures available for children to use. A sampling of children and staff records were reviewed. LPA reviewed 5 children's files which were found to be complete and current. Staff mandated reporter certification are all current. LPA verified SB792 Child Care Adult Immunization and Tuberculosis requirements. CPR/ First aid certifications are up to date, experiencing 07/2025. Continued on 809-C

Site Supervisor was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. A Technical Violation was issued for not having written instructions on how to administer over the counter lotion including times. Lotion also needs to be labeled with child's name.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Exit interview conducted, appeal rights were given, and report was reviewed with site supervisor Ashley Rodriguez.
