A Case Management Inspection was conducted by Licensing Program Analysts (LPAs) S. Mendoza-Ceja and B. Cervantes who met with the Director Grace Montejano. The center was toured inside and outside.

During the inspection, it was found that Victoria Neal was not associated to this center; however,
Victoria Neal does have a criminal record clearance with the Department and has a valid Child Development Site Supervisor Permit. LPAs advised failure to obtain the criminal record clearance association for Victoria Neal with this center has resulted in an immediate civil penalty of $500.000 assessed.

The following Type B deficiency is cited on page 2, according to CCR, Title 22, Division 12 in regards to the criminal record clearances. Appeal Rights reviewed.
