On May 31 2024 at 9:20 AM, Licensing Program Analysts (LPAs) Aaliyah Zendejas and David Roman conducted an unannounced Annual/Random Inspection at the above mentioned Child Care Center (CCC). LPAs met with Faviola Ambriz (S1), staff member at the CCC and explained the purpose of the inspection. S1 stated that the site supervisor was not present today, but that they can assist LPAs for the day. LPA checked designation of responsibility and found the correct form completed for S1. LPAs toured the interior and exterior of the CCC with S1.

LPA observed required licensing documents mounted on the walls throughout the facility. Last fire drill was conducted on 4/30/2024. The classroom has age-appropriate toys and furniture readily accessible for children in care. LPAs observed enough restrooms available for children to use. LPA did not observe any hazards/toxins items accessible to children. A first aid kit was observed within the facility in a locked cabinet. The outdoor playground is shaded and has an ample amount of space for children to play.



LPA observed the playground has age-appropriate toys and structures available for children to use. LPA asked to see the sign in and sign out sheets for children in care, and S1 showed that they did it online on a website called hubbe. LPA checked to make sure that all children were accounted for which they were.

A sampling of children and staff records were reviewed. LPA observed children's files to be complete and current. Currently the facility has 2 children that require Incidental Medical services (IMS). LPA observed medications stored out of reach from children with correct paperwork completed in locked storage cabinet. LPA observed staff files to be complete and current. CPR current with an expiration date of April 22, 2025. Staff mandated reporter certification are all current and placed within the file.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.



No deficiencies were cited.

A notice of site visit was given and must remain posted for 30 days.

Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Exit interview conducted, appeal rights were given, and report was reviewed with lead teacher, Faviola Ambriz.
