25 | On November 10, 2021 at 1:00PM, Licensing Program Analyst (LPA) Betzayra Cervantes made an unannounced visit to conduct Case Management - Incident inspection. LPA met with Site Supervisor Patricia Patchett and discussed the nature and purpose of the visit and together a tour of the facility was conducted both inside and outside. LPA observed a total of 5 children under the care and supervision of 3 teachers including the Site Supervisor.
On 11/03/2021, the Site Supervisor self reported to Community Care Licensing (CCL) an incident that occurred on 10/29/21 at or around 9:00AM where Child #1 sustained an injury to the chin requiring stiches. There were 3 children under the care and supervisor of 3 staff. The children were playing outside on the playground. Child #1 and Child #2 were playing together using the wobble chairs as barrels, rolling them along the playground. C2 ended up colliding into C1, causing him to hit his chin on the plastic chair. C1's chin was bleeding and staff immediately provided first aid and applied ice to the injury. The facility immediately contacted the parent who arrived at 9:40AM to pick up C1 to take him to get medical attention. C1 was given stiches and no other restrictions were imposed. According to the Site Supervisor, C1 returned to the center the following day on 11/01/21 and advised the child has been doing well and playing with the other children.
Site Supervisor reviewed safety practices and expectations with staff and reminded children on how to play safely and use the equipment. Based on the information obtained from the Site Supervisor as well as the LPA's observations, LPA determined there were no deficiencies and that CCC functioned in accordance with Title 22 regulations.
No deficiencies were cited during today's visit. Exit interview conducted and report was reviewed with the Site Supervisor, Patricia Patchett. |
